Important Information For Parents

Dear Camp Chi Parent,

There’s no better way to beat the winter blues than to think about summer.  We’re here to help by providing this year’s camp forms.  Your cooperation with returning completed forms early will help us provide the best possible camp experience for your child.

Please understand that each camper form is reviewed extensively by senior staff members and that we take this process very seriously. Each summer we take responsibility for hundreds of sons and daughters. Your child is our child during their stay at camp. It is crucial, for this reason, that we receive your camper forms as soon as possible. This is the only way we have to get to know your camper, and his/her needs, BEFORE arrival day.

We understand that it may take some time to schedule a physical for your child, please schedule this doctor's appointment today.

All camper and medical forms are due by March 1, 2010, in order for your child to attend camp.

Program Permission (signed by parent)

Camper Behavior Contract (signed by parent and camper)

Friendship Request (completed by parent and campers in 4th-10th grades)

Medical Form Part 1 (3 pages completed and signed by parent)

Medical Form Part 2 (completed and signed by medical professional)

Optional Forms

B'nai Mitzvah Tutoring (only for campers celebrating from Aug-Oct '10)

Pacific Northwest Wilderness Permission Letter and Form
